嚢胞状水腎杯を呈した腎細胞癌の1例

A CASE OF RENAL CELL CARCINOMA PRESENTING WITH CYST-LIKE HYDROCALYX

　74歳，女性が左季肋部痛と腹部腫瘤を訴え入院した。IVP, CT，腎動脈造影にて左腎上部に巨人な嚢胞があり腎実質と嚢胞の移行部に腫瘍の存在が疑われたため，腎腫瘍と嚢胞の合併と診断し左根治的腎摘除術を施行した。腫瘍は病理学的には明細胞癌であつた。また嚢胞の内面には一部移行上皮が存在しており，われわれは，嚢胞は腫瘍によつて腎杯頸部の閉塞が起こつたための嚢胞様水腎杯であると推測した。

A 74-year-old woman complained of left hypochondralgia and abdominal mass. CT showed a large cyst in the upper portion of the left kidney with irregular inner wall. Angiography revealed a tumor stain adjacent to the cyst caudally. The patient underwent an uneventful left radical nephrectomy. The cyst contained 1210 ml of yellow serous fluid. Cut section of the removed kidney showed a yellow mass adjacent to the cyst. The inner surface of the cyst was studded with small whitish yellow lumps.
