経皮的腎尿管切石術60例の経験

PERCUTANEOUS NEPHROURETEROLITHOTOMY (PNL): CLINICAL EXPERIENCE OF SIXTY CASES

　過去1年間に16例の尿管結石と44例の腎結石に対し経皮的腎尿管切石術（PNL）を施行した。90％の症例に腎杯穿刺前に6Fr．UPJ-occlusion balloon catheterを患側尿管に留置したが，これは特に尿管結石では結石を腎盂内に吹き上げるのに有効であつた。残石率は術後CTスキャンでのみ観察可能な小結石を含め14％であつたが，臨床的には93.4％に満足しうる結果が得られた。PNL術中の腎盂内圧を測定したところ，Amplatz workingsheathを使用しない場合には40〜60cmH₂Oと高く，灌流液の体内吸収を考慮して術中の中心静脈圧の監視が必要であると考えられた。

Since October 1984, 60 patients have undergone percutaneous stone extraction. With construction of the percutaneous nephrostomy tract as the conduit to the urinary tract, we have removed 93.8% of the upper two third ureteral stones, 96.1% of the single calyceal or pelvic stones and 62.5% of the complex pelviocalyceal stones. The overall clinical success rate was 93.4%. The intrapelvic pressure measured through the lumen of UPJ occlusion balloon catheter was kept fairly low (less than 10 cm H₂O), when PNL was performed with 30 F Amplatz working sheath.
