器質的インポテンスに対するsilicone-silver penile prosthesis挿入手術の経験

EXPERIENCE WITH SILICONE-SILVER PENILE PROSTHESIS IN PATIENTS WITH ORGANIC IMPOTENCE

緒言

　インポテンス（以下IMP）の治療として両側の陰茎海綿体白膜下に一対のpenile prosthesisを挿入し勃起状態を維持させる方法は1966年にBeheri^1）によつて初めて報告され，prosthesis挿入手術として現在最も広く行われている手術法の原型とされている。その後，材質と手術法について研究が積み重ねられた結果，Scott型^2）のinfla-table penile prosthesisとSmall Carrion型^3），Finney型^4），Jonas型^5）のnoninflatable penileprosthesisが実用化され，その安全性と有用性が評価されている。本邦においても岡本ら^6），藤岡ら^7），石井ら^8）による報告があるが未だ普及した治療法とは言い難い。著者らは1984年8月から1985年7月までの1年間に8例の患者にPenileprosthesis挿入手術を施行し，その安全性と治療成績を検討したので報告する。

Between August 1984 and July 1985 a silicone-silver penile prosthesis was inserted in 8 patients with organic impotence. Etiologies of the impotence were radical surgery for bladder cancer or rectum cancer in 4 patients, spinal cord injury in 3 patients and unknown in a 74 years old patient. During the follow-up periods ranged from 3 months to 14 months, there was no serious complication or removal of implants. Later than 3 weeks following operation, all the patients reported excellent results both func-tionally and psychologically.
