針生検で発見された前立腺マラコプラキア

PROSTATIC MALACOPLAKIA DETECTED BY NEEDLE BIOPSY

　マラコプラキアは，非特異性肉芽腫性炎症で尿路に好発するが，他の種々の臓器にも発生しうる。Von Hansemann細胞やMichaelis-Gutmann小体（M-G小体）が特徴であり，マクロファージの病変とされているが，免疫学的にも問題のある疾患である。

　前立腺マラコプラキアは，最近は報告例が増加しており，しかも生検で発見された例が多い。われわれも針生検にて前立腺マラコプラキアと診断された1例を経験したので報告する。

A 50-year-old man complained of high fever and right side lumbago without any sign of dysuria.

The urine specimen showed many leucocytes and E. coli of 10⁷/ml. Rectal examination revealed a firm prostate with enlargement on the left lobe. Several specimens from this prostate by needle biopsy was proved prostatic malacoplakia. He was treated by antibiotics combined with cholinergic agents and vitamine C. The convalescence was uneventful. The clinical features, histopathology and treatment were shortly described.
