前立腺針生検の臨床的検討

A Review of 193 Prostatic Needle Biopsies

　当科における前立腺生検193例につき，前立腺癌診断上の問題点を検討した。1993年1月より1996年6月までの間に，主として下部尿路症状を訴えて当科を受診した症例のうち，直腸診，前立腺特異抗原高値などにより前立腺癌を疑った193例に生検を行った。生検で60例に前立腺癌の診断がなされた。さらに初回の生検で癌を認めなかった症例のうち72例に経尿道的前立腺切除術を行い，5例に癌を認めた。直腸診単独，前立腺特異抗原単独，直腸診＋前立腺特異抗原のそれぞれによる陽性予測値は9.1％，15.7％，70.3％であった。

From January, 1993 to June, 1996, the patients who visitedour hospital with a chief complaint of lower urinary tractsymptoms were evaluated with digital rectal examination(DRE), serum prostate specific antigen (PSA). In 193patients, mainly transperineal prostatic needle biopsieswere done using the automatic biopsy gun fitted with an18gauge biopsy needle under lumbar anesthesia.
