血清PSA値が高値を示した前立腺マラコプラキア

A case of prostatic malacoplakia showing increased serum PSA value

69歳，男性。高熱と排尿困難を主訴に近医を受診し，膿尿が認められ尿路感染症の診断で抗生剤投与が行われ，すみやかに軽快したが，PSAが111.0ng/mlにて当科に紹介された。直腸診で前立腺は硬く，前立腺癌疑いで針生検を施行し，前立腺マラコプラキアと診断された。生検1か月後にはPSAは5.7ng/mlまで低下しており，前立腺炎との合併が疑われた。

A 69-year old man with complaints of high fever and dysuria presented showing pyuria and increased serum PSA value up to 111.0ng/ml. After the administration of antibiotics for several days,the urine became sterile and the symptoms disappeared,but rectal digital examination revealed a firm prostate with the suspicion of prostatic cancer. Examination of transperineal needle biopsy of the prostate revealed the histopathological features of malacoplakia. The serum PSA decreased to 5.7ng/ml one month later. The patient urinated with good stream and follow-up urinalysis was normal. Prostatic malacoplakia with high PSA value is briefly reviewed.
