前立腺自動生検針の有用性

TRANSRECTAL PROSTATIC NEEDLE BIOPSY WITH BIOPSY-GUN FOR DIAGNOSIS OF PROSTATIC CANCER

　直腸診で軽度の前立腺肥大症以外の異常所見を前立腺部に認めたとき，自動穿刺装置を用いて経直腸式前立腺生検を行った．異常所見を認めた115症例中17例に前立腺癌を認めた．17例の前立腺癌のうち9例がstage B,1例がstage C,7例がstage Dであった．合併症は直腸出血を2例，急性前立腺炎を2例認めた．自動穿刺装置を用いた経直腸式生検は今まで行ってきた経会陰式生検と異なり，外来患者にほとんど苦痛を与えることなく，無麻酔，無処置で施行でき，異常所見の部位を正確に穿刺でき，瞬時に組織挫滅の少ない標本を採取できた．自動穿刺装置を用いた経直腸式前立腺生検は日常外来診療における前立腺癌患者の診断に有用な検査法と思われる．

Transrectal prostatic needle biopsies were performed in outpatients with palpable prostatic abnormal-ities using two types of automatic, spring-powered needle biopsy device (biopsy-gun). Seventeen patients were revealed to have prostatic cancer out of 115 outpatients who underwent the biopsy. Nine out of 17 patients with prostatic cancer were diagnosed as having clinical stage B. One patient had stage C. Seven had stage D. Side effects were rectal bleeding (two patients) and acute prostatitis (two patients).
