前部尿道狭窄に対するsleeve urethroplastyの経験

EXPERIENCES ON THE SLEEVE URETHROPLASTY METHOD

　内視鏡の進歩により，初心者にも安全に行えるようになつた直視下内尿道切開術は，現在では，尿道狭窄治療の主流となりつつある。しかし，瘢痕を残す手術であるので適応，治療成績にも限界があり，成功率は100％とはならない。骨盤骨折による膜様部尿道狭窄，瘻孔形成のあるもの，瘢痕形成の強いものは手術治療が必要となる。狭窄範囲の長い，瘢痕形成の強い前部尿道狭窄の2症例に対し，sleeve urethroplastyを行つたのでその経験を述べる。

Experiences on the sleeve urethroplasty method were reported. Following retraction of the skin around a penile shaft just like a sleeve, a long anterior urethral stricture was incised longitudinally and was left open. After urethral catheter was indwelled, the penile skin was pulled forward and the wound was closed. Two cases were treated successfully by this sleeve urethroplasty method. No recurrence of stricture was noted at postoperative 5 and 2 years respectively. This method is recommenced on a case with long anterior urethral stricture refractory to usual internal urethrotomy either by blind or direct vision method.
