先天性直腸尿道瘻の2例

CONGENITAL RECTOURETHRAL FISTULA: REPORT OF 2 CASES

緒言

　5,6年前，尿道狭窄の手術に興味を持ち始めてから，直腸尿道瘻の症例にみずからの手で当たつてみたいと考えていた。大変むずかしい手術のように文献から印象を受けていた。

　入局当時，恩師原田教授が会陰式前立腺摘除術後の本症に，大変手こずられたのを記憶している。自分自身では，6年前の散弾による直腸尿道瘻の症例および約15年前の膀胱全摘出術時の直腸損傷に対して，Young-Stoneの手術をした経験があるが，いずれも緊急手術であらかじめプランを立てて，案を練つてやつた手術ではない。

2 cases of congenital recto-urethral fistula were reported. Perineal closure of the urethra and rectum in 2 layers was done after colostomy and suprapubic cystostomy with success, in the first case. Perineal closure and suprapubic cystostomy without colostomy lead to good result inspite of infection in the second case.

Rectal palpation is important for diagnosis and would like to stress its importance as a guide during surgery. Complete 2 layer closure of the rectal wall with setaining urethral catheter could be the pro-cedure whenever there is a possibility of urethral stricture.
