尿道狭窄に対するIsland Patch Urethroplastyの3例

ISLAND PATCH URETHROPLASTY FOR URETHRAL STRICTURE: EXPERIENCE WITH THREE PATIENTS

はじめに

　球部尿道狭窄の3症例に対し，one-stage opera-tionであるisland patch urethroplasyを施行し，満足すべき成績を得たので，尿道形成術に関し若干の文献的考察を加え報告する。

　本術式は振子部から膜様部までのあらゆる長さの尿道狭窄に対し利用しうる術式である。最初振子部狭窄に対する術式としてLeadbetter　（1962）^1），Orandi　（1972）^2），Anastasi　（1973）^3）が発表し，球部から膜様部狭窄にも応用しうるものにBlandy（1974）^4）が発展させた。第1図にその模式図を示す。陰嚢皮膚より肉様膜を茎とする有茎陰嚢皮弁を形成し，その表皮側が尿道内腔に向くようにその皮弁をpatchとして切開された狭窄部尿道にあてるというものである。振子部および球部尿道狭窄に対する手術術式の相違点は皮膚切開線のみである。術式の詳細については引用文献^1〜4）を参照されたい。

Good results obtained in island patch urethroplasty were reported, using a pedicled scrotal flap developed by Blandy et al in 3 cases of bulbar urethral stricture. Their ages were 44, 70 and 72 years, and the length of stricture were 0.5, 3 and 2.5 cm respectively. The post-operative follow-up period was 6 to 12 months. Except 1 case accompanied with neurogenic bladder, pyuria was not noted, and good micturition was possible. The case with neurogenic bladder showed about 50 ml residual urine volume even after the operation, but there was no problem at all in urethral catheterization.
