膀胱子宮頸管瘻の1例

A CASE OF VESICO-CERVICAL FISTULA

はじめに

　膀胱と女性性器との間に生じる瘻孔は大多数が膀胱腟瘻で，膀胱子宮瘻は比較的稀である。われわれは，帝王切開術のあと1年7カ月後に吸引分娩で出産した直後から膀胱子宮瘻を発生した症例を経験したので報告し，本邦報告61例を集計して若干の考察を加えてみる。

A 31-year-old housewife had gross hematuria immediately after vacuum extraction delivery and noticed urinary leakage from the vagina a few days later. One year and 7 months before this delivery she had a cesarean section. She continued to have urinary incontinence and periodic hematuria in the following 8 months. Cystoscopic examination revealed a small fistula above the right ureteral orifice and hystero-graphy showed a fistula between the bladder and uterine cervix. Transperitoneal closure of the fistula was performed with excellent result. The patient was discharged 2 weeks after operation, and has since been asymptomatic.
