睾丸腫瘍の組織型診断における血中HCGおよびAFP測定の意義について

THE ROLE OF SERUM HCG AND AFP IN HISTOLOGICAL DIAGNOSIS OF TESTICULAR GERM CELL TUMOR

はじめに

　胚細胞性睾丸腫瘍（以下睾丸腫瘍とする）の組織型は，大きく精上皮腫と非精上皮腫に分類され，両者は悪性度も有効な治療法も異なる。両者の鑑別は摘出された原発巣の組織学的検索によりなされるが，常にsampling errorの可能性を伴い，原発巣が精上皮腫であつても転移巣が非精上皮腫である可能性は捨て切れない。

　human chorionic gonadotropin（HCG）とα-fetoprotein（AFP）は，radioimmunoassay（RIA）に代表される微量測定法の開発に伴い，睾丸腫瘍のmarkerとして，その診断，治療効果判定および経過観察に広く利用されるに至つた。

The levels of human chorionic gonadotropin (HCG), beta subunit of HCG (HCG-beta) and alpha-fetoprotein (AFP) were determined using radioimmunoassay in sera and tumor extracts of the patients with testicular germ cell tumor to evaluate the role of these markers in its histological diagnosis.

Elevation of serum AFP indicated non-seminoma and positive serum HCG indicated non-seminoma or seminoma containing syncytiotrophoblastic giant cells (STGC). A substance with immunological similarity to free HCG-beta was detected commonly in tumor extracts of the patients with pure semi-noma and frequently in their sera.
