腎腫瘍を疑わせた後腹膜膿瘍の1例

RETROPERITONEAL ABSCESS SUSPECTED OF RENAL TUMOR: A CASE REPORT

緒言

　泌尿器科領域における後腹膜膿瘍は，その対象とする臓器の位置関係から分類上posterior abscessに属するが，早期に正確な診断を下すことは比較的難しく，治療が外科的ドレナージに頼らざるを得ないことなど臨床的には必ずしも容易な疾患ではない。著者らは，腎腫瘍を強く疑わせる臨床像を呈した，糖尿病を伴つた後腹膜膿瘍の1例を経験したので，ここに報告する。

A 44-year-old woman was admitted with chief complaints of high fever and 1t. flank dull pain. As mild diabetes mellitus was revealed by 50g GTT and Klebsiella pneumoniae was detected by arterial blood culture, control of diabetes mellitus using insulin and administration of antibitics sensitive to klebsiella was started. At the same time, routine radiological examinations including IVP, renal angio-graphy and CT scanning indicated the diagnosis of 1t. renal tumor. Transpertioneal nephrectomy was intended but not performed because massive pus was discharged from retroperitoneal space without any evidence of renal tumor.
