A 44-year-old man with hypertension of sudden onset and with unilateral contracted kidney under-went nephrectomy which was followed by a rapid fall in PRA and normalization of blood pressure. A particular finding in the removed kidney was slight hyperplasia of juxtaglomerular cells which had characteristics similar to the cells producing renin. Sections of the main renal artery showed slight arterial thickening, but arteriolar vessels showed striking intimal proliferation and transmural fibrosis.
