Two cases of suspected unilateral adrenal agenesis, one in a 39 year-old female with agenesis of right kidney and adrenal showing Cushing's syndrome from adenoma of left adrenal and another case in a 32 year old male with agenesis of left adrenal and anastomosis of right adrenal vein and liver, were reported. Atrophic, accessory adrenal and adrenal-renal heterotopia must be considered for differential diagnosis and long term ACTH treatment was recomended for these cases, after bilateral adrenalectomy, to accelerate the function of the accessory adrenal.
