腎外傷200例の臨床的観察

RENAL TRAUMA: AN ANALYSIS OF 200 RENAL INJURIES

緒言

　腎臓は後面は腸腰筋などの筋肉群に，また前面は腹腔内臓器により保護され，後腹膜腔に存するが，腎外傷は腹部外傷のうちでも比較的多いとされている^1）。そして，交通事情が激しく，また複雑化してきている現代社会では，交通事故，労災事故などの不慮の事故が多くなつてきており，泌尿器科領域でも外傷学は重要な一分野である。そこで，われわれは過去の経験に基づき，救急医学の一助とするべく，最近までの200例の腎外傷について臨床的観察を行なつたので，ここに報告する。

The clinical observation was made on 200 renal injuries in our clinic during the 12 years period from 1966 to 1977. The incidence of renal trauma was 23.9% in all of 835 patients with abdominal injuries. The most frequent age distribution (44% of cases) was between 21 and 40 years old. There were 172 males and 28 females. A variety of cases included traffic accident (60%), other blunt trauma (34.5%), stab wounds (3%) and others (2.5%). Associated injuries were present in 66.5% of the patients, including intra-abdominal injuries (36.5%), thoracic injuries (28.5%), head injuries (12.5%) and pelvic fractures (8%).
