副腎腫瘍と鑑別困難であった後腹膜脂肪肉腫

Retroperitoneal Liposarcoma Presenting Similar Clinical Findings of Adrenal Tumor:A Case Report

　症例は57歳，男性。顕微鏡的血尿を主訴に受診した。IVPで左腎に占拠性病変を疑わせる所見があり，CTを施行し，左副腎腫瘍が疑われた。血管造影にて左副腎腫瘍と診断し，左副腎摘出術を施行した。病理組織学的には分化型の脂肪肉腫と診断され，後腹膜脂肪肉腫が副腎へ浸潤したものと考えられた。術後，補助化学療法としてCYVADIC療法を1クール施行した。経過は良好で，術後4年3か月経過した現在まで再発，転移を認めていない。

A 57 years-old male was referred to our clinic for the evaluation of microscopic hematuria. An excretory urogram showed a suspicious space occupying lesion of the left renal upper pole. However, computerized tomography and an-giography revealed a left adrenal tumor. The left adrena-lectomy was done and the round shaped specimen was 6cm in diameter. The histopathological diagnosis was well dif-ferentiated liposarcoma in the retroperitoneum which was invaded to the adrenal gland. Postoperative adjuvant che-motherapy conisting of cyclophosphamide, vincrystine, adriamycin and dacarbazine was performed.
