腎癌に合併した高カルシウム血症の臨床的検討

HYPERCALCEMIA ASSOCIATED WITH RENAL CELL CALCINOMA

緒言

　悪性腫瘍に高Ca血症を伴うことはまれではない。とくに腎癌は肺癌とならんで高Ca血症を起こしやすい悪性腫瘍の一つで，その成因も1）骨転移による骨破壊，2）高Ca血症を来す物質の腫瘍内産生，3）原発性副甲状腺機能亢進症のような良性疾患の合併，4）薬剤の過剰投与などさまざまである。

　今回，われわれは当教室で経験した腎癌に伴つた高Ca血症の8例をもとにして，高Ca血症の成因に関する鑑別診断とその治療法について臨床的検討を加えたので報告する。

Eight cases of hypercalcemia associated with renal cell carcinoma were reported. The mechanism of hypercalcemia in these cases were the ectopic production of hormone-like substance (4 cases), bone metastasis (3 cases) and the association of primary hyperparathyroidism (1 case). In the cases of the ectopic production of hormone-like substance, the products could not be analysed. The differential diagnosis of the mechanism of hypercalcemia was assessed by the history of hypercalcemia before the development of cancer, the effect of cancer therapy and the venous sampling for parathyroid hormone.
