後腹膜病変の鑑別診断におけるCT （Computed Tomography）の役割

DIAGNOSIS OF RETROPERITONEAL MASS LESIONS BY COMPUTED TOMOGRAHY

緒言

　泌尿器科領域における全身用Computed Tomo-graphy（以下CTと略す）の有用性に関しては，すでにいくつかの報告が見うけられる^1〜3）。これらの報告を総合すると，CTの有用性として次のような点が挙げられる。1）従来のX線検査では描出できなかつた病変を映像化する。2）病変の内部構造を明らかにする。3）病変の水平方向の拡がりを知り，従来のX線検査とあわせた立体的な情報から，治療方針の選定や治療効果の判定に用いる。4）逆行性腎盂撮影，血管撮影あるいは後腹膜送気法などの侵襲の多い検査を省略する。

　ところで，以上のような利点に加えてCTが広く普及し精度も向上した現在では，CTを日常検査の1つと考えて特殊な病変のスクリーニングに使うことも可能である。著者らは今回，従来のX線検査では発見が困難とされていた後腹膜病変の検索にあたり，CTを静脈性腎盂造影（以下IVPと略す）につぐスクリーニング検査として施行した結果，興味ある症例を発見し得たので報告する。

The displacement of the kidney or ureter suggested by excretory urography is often encountered, presenting a difficult problems in diagnosis. A retroperitoneal mass must be differentiated from the disease of the liver, spleen and kidney itself. Nine cases of retroperitoneal mass were studied by CT scan, including five cases of primary retroperitoneal tumor, three absess and one hematoma. A correct preoperative diagnosis with topographic relationship to the adjacent structure was made in eight of these cases examined with this modality.
