超選択的膀胱動脈造影が診断に有用であつた尿膜管腫瘍の1例

CARCINOMA OF THE URACHUS: VALUE OF THE SUPER-SELECTIVE VESICAL ARTERIOGRAPHY

緒言

　尿膜管腫瘍は本邦ですでに150例の報告があり，もはやまれな疾患とは言い難い。しかし，早期診断が困難なため，根治手術の適応にならない症例が多い。著者らは膀胱鏡検査にて尿膜管腫瘍を疑い，超選択的膀胱動脈造影により術前に診断を確定し根治手術を行ない順調に経過している症例を経験したので報告する。

A case of 45-year-old man with carcinoma of the urachus was presented. His chief complaint was total, painless gross, hematuria. Cystoscopy disclosed a sessile tumor with smooth surface at the dome of the bladder. Bimanual examination failed to palpate abdominal masses and there was no evidence of tumor metastases in X-rays, gallium scintigrams and other investigations. Classical pelvic angiography did not show any abnormalities but super-selective vesical arteriography revealed an extra-vesical mass exten-ding towards the umbilicus, which was hypovascular.
