遺残尿膜管に合併した腹壁膿瘍の1例

A CASE OF ABDOMINAL WALL ABSCESS ASSOCIATED WITH THE REMNANT URACHUS

　症例は21歳の男性。下腹部腫瘤，頻尿を主訴に来院。初診時に下腹部正中線上に圧痛を伴う手拳大の腫瘤を触知した。CTスキャンにて臍下部の腹壁に膿瘍腔と膀胱頂部に尿膜管遺残と思われる腫瘤を認めた。感染をおこした遺残尿膜管の腹壁への穿破による腹壁膿瘍と考え，膿瘍の切開排膿を行つた後に二期的に膀胱部分切除を含めた尿膜管全摘術を行つた。

A 21-year-old man presented with urinary frequency and a mass in the lower abdomen. Physical examination revealed a tender fist-size mass just below the navel. Urine examination showed pyuria with E. coli infection. CT scan disclosed the low density area located anterior to the rectus abdominis muscle. CT scan also disclosed a cystic mass at the dome of the bladder, which was compatible with the rem-nant urachus.
