尿細胞診の臨床統計

STATISTICAL OBSERVATION OF EXFOLIATIVE URINARY CYTOLOGY

はじめに

　膀胱癌における尿細胞診の有用性については，膀胱上皮内癌（carcinoma in situ）の診断，膀胱癌再発のscreeningなどですでに認められている。

　また，診断困難な症例に遭遇する機会が多い上部尿路腫瘍，とくに尿管腫瘍の診断上「尿細胞診の意義」を経験した泌尿器科医も多いことであろう。

358 consecutive urinary cytologic examinations done between 1970 and 1978 were studied retropec-tively in 182 patients with genitourinary complaints seen at Nippon Medical School.

There were 44 proven bladder carcinoma of which 26 were associated with abnormal smears (class Ⅲ_b, Ⅳ, Ⅴ) and 18 with negative smears (class Ⅰ, Ⅱ, Ⅲ_a), and 14 proven renal pelvic and ureteral carcinoma of which only 5 were associated with abnormal smears.

Diagnostic accuracy at present is still limited, however, with improved technique it might become possible to detect malignancies in earlier stage.
