経尿道的前立腺切除術の輸液，マンニトール添加ラクテートリンゲル液の使用経験

INTRAVENOUS INFUSION OF 2.5% MANNITOL IN LACTATED RINGER'S SOLUTION FOR TRANSURETHRAL PRESTATIC RESECTION

緒言

　経尿道的前立腺切除（TUR-P）は侵襲の少ない手術とされているが，その施行にあたり手術手技は必ずしも容易ではなく，加うるに高齢者が対象となつているために，経験の浅い者が術者となつた場合には術中思わぬ合併症を併発することがある。特に手術時間の延長は術中の血圧変動の誘因となると同時に，血中電解質や術後尿量に変化を及ぼすものである。今回筆者らはこれらの点を加味し術中術後の管理をより容易にせしめる輸液として，2.5％マンニトール添加ラクテートリンゲル液（M-L液）を試作使用し良好な成績を得たので報告する。

For intra- and post-operative management of TUR-p patients, the effect of infusion of 2.5% man-nitol in lactated Ringer's solusion (osmolality 346.2 mOsm/ι) was investigated clinically in 20 cases. These procedures were done under the low spinal anesthesia (saddle block). The mean resecting time was 43.7±10.2 munites and the mean weight of the resected prostate was 30.4+10.7gm. No blood transfusion was needed. During TUR_-p, the blood pressure was stable without the use of vasopressors and the diuretic effect of mannitol appeared in the post-operative period.
