経尿道的膀胱腫瘍切除術施行時のmultiple random mucosal biopsyの意義—特に上皮内癌の診断に関して

MULTIPLE RANDOM MUCOSAL BIOPSY WITH TRANSURETHRAL RESECTION OF BLADDER TUMOR: DIAGNOSIS OF CARCINOMA IN SITU

緒言

　膀胱上皮内癌（carcinoma in situ）は表在癌でありながら早期に浸潤癌に移行する危険性があり，かつ広範で不規則な広がりを示すことが多く近年その存在が注目されている^1）。しかもその膀胱鏡所見は特異像を認めず多くの症例において潜在した癌として発見されている。

　著者らは今回22例にmultiple random mucosal　biopsyを施行し，うち7例に上皮内癌を確認し，これら7例全例に膀胱全摘除を行なつた。これら症例において内視鏡所見を初めとする術前検査，生検および膀胱摘除標本の病理診断とを比較することにより若干の知見を得たので報告する。

Carcinoma in situ of the bladder is superficial low stage, has a possibility of developing to an invasive cancer in a short time and spreads irregularly. That carcinoma is diagnosed as a latent cancer because of a lack of specific finding of cystoscopy.

We have performed multiple random mucosal biopsy as cold punch technique in 22 cases at St. luke's Hospital since 1979. TUR-Bt was done in 20 cases out of the total at the same time. In 7 cases, the biopsy revealed carcinoma in situ at the tumor free mucosa by endoscopy and radical cystectomy was performed in all 7 cases.
