緒言

　経尿道的前立腺切除（TUR-P）は侵襲の少ない手術とされているが，諸主要臓器機能の低下を来している老齢者を対象としており，また出血やTUR反応などの問題があるために術中の管理には十分な注意が必要である^1）。当科では600例を越えるTURを行ない，その間により安全な管理をめざして種々な検討を加えてきたが，今回マニトールを主成分とする輸液に塩酸エフェドリンを加えた製剤を作成して術中輸液に用いてよい結果が得られるようになつたので報告する。

The effect of several infusion fluids on the blood pressure during transurethral prostatectomy was evaluated. A bottle of 200ml lactate Ringer's solution was prepared in our hospital with mannitol and ephedrine hydrochloride. In the initial phase after epidural anesthesia this solution was infused. The speed of infusion was adjusted according to the level of the blood pressure. Within ten or twenty minutes, the infusion was switched to the solution including mannitol without ephedrine hydrochloride. The results were excellent. The solution may also be useful for the management of vasodilation shock preventing renal failure.
