TWO CASES OF URETERAL OBSTRUCTION TREATED WITH GIBBONS INDWELLING URETERAL CATHETERS

緒言

　尿管の通過障害に比較的encrustation，石灰化および異物反応の生じにくいシリコン製の尿管カテーテルを長期にわたり留置することは以前から試みられていたが^1〜5），カテーテルを尿管内に定着させにくいという主問題点があつた。

　最近Gibbons^6）らは，カテーテルに尿管の蠕動運動に抵抗するように中間部に棘突起を，また尿管口側の末端部にflangeを工夫することにより，尿管内での定着性を持たせることに成功した。さらに，このGibbons stentはシリコンの中にスプリング・コイルを埋蔵し，外力に対して抵抗性で，腎盂側の先端はレントゲン非透過性であるなどの利点をも有する（第1図上）。

Two cases of ureteral obstruction treated with Gibbons stent were reported. First case was a 52-year-old female who had radiotherapy for an inoperable uterus carcinoma followed by obstructive anuria. Gibbons stent was inserted to the left ureter. Although satisfactory urine volume was obtained at the biginning, it was soon obstructed and was removed. Second case was a 61-year-old male who had a diagnosis of urinary tract tuberculosis and IVP revealed right ureteropyelocaliectasis. Gibbons stent was passed up to the renal pelvis prior to antituberculosis chemotherapy in order to prevent further stenosis of the ureter.
