成人女子の膀胱外反症2例

EXTROPHY OF THE BLADDER:TWO ADULT CASES

緒言

　小児の泌尿器奇型の膀胱外反症は比較的稀な疾患であるが，他臓器奇型を合併することが多く，また尿路感染を起こしやすいことが原因で，予後は必ずしも良くない。最近われわれは26歳と25歳の女子の膀胱外反症2例を経験し，手術的治療を施行したので報告する。

Case 1. A 26 years old female was seen complaining of a lower abdominal tumor and urinary incontinence since birth. Examination revealed an extrophic bladder, bilateral hydronephroureter (Fig. 2) and widely separated rami of pubic bones. She could pass some amount of urine voluntarily from ureteral orifices with abdominal strain (Fig. 1). She underwent cystectomy and construction of ileal conduit. Postoperative course was uneventful.

Case 2. A 25 years old female was seen because of urinary incontinence since birth. Examination revealed an extrophic bladder (Fig. 4).
