無血管性腎細胞癌の1例

A CASE OF AVASCULAR RENAL TUMOR WITH MASSIVE NECROSIS

緒言

　腎癌の術前診断には従来，IVP，尿細胞診，血管撮影などが用いられていたが，最近ではCT，超音波断層法（エコー）が加わり，さらに超音波下にて嚢胞穿刺も行えるようになつて確定診断が比較的容易になつた。しかし，血管撮影にてavascularityを示し腎嚢胞を強く疑わせ，腎腫瘍との鑑別が困難なため，術式を含めた治療の選択に苦慮することがある。

　最近，われわれはこのような血管撮影上腎嚢胞を疑つたavascular renal tumorの1例を経験したので報告する。

A 57-year-old man was admitted to our hospital complaining of right flank pain and gross hematuria.

Intravenous pyelography showed the depressed upper calyx of the right kidney, suggesting a space occupying lesion. Renal angiography including pharmacoangiography revealed an avascular and well-defined lesion. These findings were compatible with benign renal cyst. However, the ultra-sonographic and computed tomographic findings suggested the solid tumor.

Since the exploration revealed a renal tumor, radical nephrectomy was performed.
