触診で硬結節を触れない前立腺癌

PROSTATIC CANCER BENIGN ON PALPATION

緒言

　前立腺癌の診断は直腸内指診によるものであり，硬い結節が触れたときに前立腺癌が考慮されるというのがどの教科書にも書いてある常識である。しかし，原則はつねに例外を伴うものであり，触診所見のみを頼りにしていると意外に誤診する機会が多い。最近では前立腺癌の3分の1は触診でわからないとさえいわれている。このような癌がすべて非活動性のものであれば臨床上問題にする必要は少ないが，実際には触診上良性疾患を思わせる前立腺のなかに，悪性度の高い癌がふくまれている。

The principal resource in the diagnosis of prostatic cancer is the rectal examination. Cancer is dete-cted as a hard nodule on palpation. Careful and detailed studies, however, show that more cancer exists in the prostate which is found to be clinically benign than previously considered. It is reported that one-third of the cases of cancer will pass unnotied even by experienced examiners. In hopes of correctly detecting many more cases of cancer, sixteen patients hardly suspected of malignancy on palpation were reviewed.
