　はじめに

　結核は，一部には過去の疾病と認識されてきたが，近年，結核罹患率が増加しており，中耳結核や咽頭・喉頭結核など耳鼻咽喉科における日常診療においても注意が必要である。

　結核の皮膚病変で，結核疹の1つであるバザン硬結性紅斑は専門医以外には鑑別が難しく，そのため結核の治療が遅れる場合がある。

　今回われわれは，数年来下腿に圧痛を伴う紅斑があり，皮膚生検の結果，バザン硬結性紅斑が疑われた症例を経験した。本例は胸部などに結核性病変がなかったため原因が不明とされ，経過中頸部リンパ節の腫大を認めたため生検を施行した。その結果，リンパ節結核と診断され，結核の治療を行い皮膚症状が軽快した。文献的考察を加えて報告する。

A 56-year-old woman developed bilateral leg erythema with tenderness since 1989. In July 1993, a skin biopsy was performed, and it revealed granula-tion with giant cells, but the diagnosis was not confirmed. Lung tuberculosis was ruled out by chest x-ray and chest CT. In November 1997, she noticeda neck tumor and was admitted to our hospital. Neck lymph node biopsy was performed and it was revealed that the source of the lymphadenopathy was tuberculosis, and we concluded that the cause of the dermatosis was erythema induratum of Bazin.

She was treated with INH, RFP, and EB, and the erythema disappeared in a month.

If erythema induratum of Bazin is suspected, we should investigate not only the lungs but also the otolaryngological areas, such as the ear, pharynx, larynx, and the neck lymph nodes.
