馬蹄鉄腎に合併したGrawitz腫瘍の1例

A CASE OF GRAWITZ'S TUMOR IN A HORSESHOE KIDNEY

緒言

　馬蹄鉄腎に他の先天性奇形や結石，水腎，結核などの二次的病変を合併しやすいことはよく知られている。しかし，腎腫瘍の合併は非常にまれで，特にGrawitz腫瘍の合併は本邦文献上，土屋ら（1957）¹），吉田（1957）²），加藤ら（1966）³），本村ら（1972）^4），鈴木ら（1974）^5）の5例を数えるのみである。最近，われわれは馬蹄鉄腎に発生したGrawitz腫瘍の1例を経験したので，若干の文献的考察を加えて報告する。

A 61 year old female visited our clinic on May 21, 1975 with the chief complaint of left flank mass. Laboratory tests revealed anemia with the RBC being 352×10⁴, advanced erythrocyte sedimentation rate of 120 mm/ hour, highly positive CRP and pyuria. IVP showed low position of both kidney, with malrotation and deformity of left renal pelvis and horse shoe kidney, fused at the lower pole, was confirmed by PRP and renal scanning. Abdominal aortogram outlined some tumor vessels at the middle part of left kidney.
