緒言

　馬蹄鉄腎に水腎症，腎盂腎炎，結核などの2次的病変が起こりやすいことは，周知のことであるが，腫瘍合併の報告は少なく，特にGrawitz腫瘍の合併は本邦文献上に土屋・豊田（1957），吉田（1958），溝口（1965）の3例のみである。最近われわれは血尿を主訴とする馬蹄鉄腎の1例で左腎に合併したGrawitz腫瘍の1例を経験したが，その診断過程において，尿細胞診は3回ともclassⅠ，大動脈撮影検査でも腎癌に特有なhypervas-cularityやpooling and puddlingも見られなかつたが，CRP陽性，貧血，血清蛋白分画の異常，血沈の亢進などのtoxic signがあるため悪性腫瘍を疑い積極的試験手術を行なうことにより，腫瘍と判明し腎剔除術を施行した1例を報告する。

A 68 year old female with chief complaint of asymptomatic hematuria visited our clinic and laboratory examination by admission, showed anemia with red blood cell count of 304×10⁴, sedimen-tation rate of 53mm/hour and serum protein fraction of α₂-Haptoglobin, γM-globulin, γG-globulin being elevated was significant. IVP showed ptosis of both kidney with malrotation and deformity of middle calyx of left kidney was observed. This part showed a ring-shaped fall out by retrograde pyelogram.
