先天性膀胱頸部通過障害の2例

CONGENITAL BLADDER NECK OBSTRUCTION: A REPORT OF TWO CASES

　幼児における下部尿路通過障害は，従来多くの文献に報告されているが，その病因の決定は必ずしも容易でなく，治療方針も報告者によつて一定していない。とくに，下部尿路通過障害としばしば同時に存在する上部尿路の変化が，前者から二次的に発生したものか，あるいは尿管開口部異常に基づく二元的なものかの鑑別は困難で，しかもこれを区別することは治療上重要なことである。今回，われわれは，手術に先立つてかなり長期間の尿道留置カテーテルを行なうことによつて，患児の全身状態を改善し，同時に有効な治療方針をたてた2例を経験したので報告し，本方針のご批判を抑ぐしだいである。

Bladder neck obstruction in young children may result from various anomalies and acquired con-tracture or hypertrophy. From the stand point of its treatment, it is very important to clarify either the upper urinary tract damage, which is often found at the same time, due to the bladder outlet obstruction or due to the obstruction at the ureterovesical junction in addition to the differential diag-nosis of the pathogenesis.
