虹彩若年性黄色肉芽腫の1例

A case of juvenile xanthogranuloma of the iris

　右眼の発赤と虹彩上に白色塊を生じた9か月の男児に，角膜混濁と，虹彩上に出血を伴った白色腫瘤がみられた。眼圧は著しく高値であった。副腎皮質ホルモン点眼薬を投与し，虹彩毛様体炎は徐々に軽減した。全身検索では，サイトメガロウイルス抗体価とリュウマトイド因子が陽性を示した以外に異常はなかった。虹彩の若年性黄色肉芽腫と診断した。若年性黄色肉芽腫は日本では稀であるが，類似した他の疾患との鑑別は重要である。

A 9-month-old male infant presented with conjunctival hyperemia and a white mass on the iris in the right eye. The cornea in the affected eye was turbid. Intraocular pressure was 46 mmHg. A whitish tumor was present on the iris with tendency for bleeding. Systemic examinations were normal except for elevated titers for cytomegalovirus and rheumatoid factor. Topical corticosteroid induced improvements of iridocyclitis. We diagnosed the condition as juvenile xanthogranuloma of the iris on clinical and laboratory features.
