腎腫瘍と誤まつた神経芽細胞腫の1例

A CASE OF NEUROBLASTOMA MISTAKEN FOR RENAL TUMOR

緒言

　神経芽細胞腫は小児悪性腫瘍のおよそ10％程度を占めWilms腫瘍とならんで頻度が高く，年齢的には2歳以下に発生したものがほぼ半数を占める。したがつて年長児や成人に発生したものは稀であり時に診断が困難となる。

　われわれは腹部腫瘤を主訴とし，他医において腎腫瘍の診断を受けた14歳女子を経過途中から診療する機会を持つたが，転医後4ヵ月目に死亡し剖検により神経芽細胞腫と判明した1例を経験したので報告する。また自験例において選択的腎動脈撮影を行なつており，その所見でも腎腫瘍を否定できなかつたが，神経芽細胞腫の動脈撮影診断およびその問題点に関し考察を試みた。また浸潤する悪性腫瘍の治療における血管内照射に関して若干の考察を加えた。

A 14 year old girl was admitted to other hospital for evaluation of left abdominal mass.

There was no history of hematuria but excretory urogram revealed slight pyelocalyectasis of the right kidney and the left kidney showed no function. Parenchymal calcification was not noticed and retrograde ureteral catheterization was unsuccessful.

Diagnosis was renal tumor and exploratory laparotomy was performed. The tumor was so huge that removal of the tumor was impossible. Histological studies by biopsy showed the findings of dark cell carcinoma.
