尿管癌の術後発生した前部尿道の乳頭状腫瘍症例

PAPILLARY TUMOR OF ANTERIOR URETHRA FOLLOWING URETERECTOMY FOR URETERAL CARCINOMA: A CASE REPORT

緒言

　膀胱腫瘍に尿道腫瘍が続発することは必ずしも稀なことではなく，Cordonnier^1），Ashworth^2）らによれば，全膀胱腫瘍の約4％にみられたと報告している。特に多発性傾向の強い膀胱腫瘍で，後部尿道にも同様な腫瘍が発生していることは度々であり，また膀胱全剔の際に後部尿道を残すと，ここに度々再発をみることもよく知られている（辻^3））。しかしながら，腎盂・尿管腫瘍の手術後同様の腫瘍が尿道，殊に前部尿道に再発した例は極めて稀なことと思われ，内外の分献にもその記載は少ない。以下，最近われわれが経験した尿管腫瘍に続発せる前部尿道移行上皮癌の1例を報告し，若干の考案を加えてみたい。

A case of anterior urethral involvement 3 years following partial ureterectomy with ureterocys-toneostomy, for transitional cell carcinoma of the ureter, in a 58-year-old male, was reported. The incidence, clinical features and treatment were discussed after reviewing the literature of related uroepi-thelial tumors. Careful follow up is mandatory for the early detection and prompt treatment for recurre-nt tumor. The importance of endoscopic examination, including urethroscopic examination of the urethra, for all uroepithelial tumors, in the routine studies for follow up, was stressed.
