膀胱腫瘍照射療法後の放射線障害（出血性膀胱炎）

HAEMORRHAGIC RADIATION CYSTITIS: A REPORT OF 3 CASES

はじめに

　膀胱腫瘍に対して，術前照射および術後照射が積極的に用いられるようになり，線源の改良，照射技術の進歩が相まつて，予後成績を著しく向上させてきた。照射効果が十分に発揮されるためには，照射に伴う副作用（骨盤腔内臓器の早期反応）を制御しながら，予定線量を完逐させることが絶対に必要であるが，一方，照射終了後に種々の形で晩期反応が出現することがあり，それが極めて難治で，時には重篤な事態を招くことがある。著者は晩期反応としての出血性膀胱炎および直腸炎の治療に難渋した経験を述べ，それらの対処の方法について考えてみたい。

Active radiation treatment for bladder tumor has been applied, resulting in improvement of the prognosis, but on the other hand, radiation hazards of the pelvic organ might appear as a incurable side effect.

Case 1 is a 27 year-old male who under went partial cystectomy for papillary carcinoma followed by Co⁶⁰ radiation (tumor dosis 5000r). Hematuria, pain on urination and anal bleeding appeared 2 months later and total cystectomy with ileal conduit for urinary diversion was done, inspite that cystoscopic examination could not distinguish the difference from infiltration of the tumor.
