高圧酸素療法が奏効したシクロホスファミドによる出血性膀胱炎

Hyperbaric oxygen therapy for cyclophosphamide-induced hemorrhagic cystitis：A case report

　症例は58歳，女性。主訴は肉眼的血尿。全身性エリテマトーデス（SLE）にてシクロホスファミド（総投与量約125g）およびプレドニゾロンを継続的に7年間内服していた。2003年8月頃より肉眼的血尿を間欠的に認めていたが放置していた。2005年7月頃より再度肉眼的血尿を認め前医で診療を受けた。同年10月になり血塊による尿閉で前医に入院した。シクロホスファミドによる出血性膀胱炎と診断され，加療目的に当科に紹介された。入院後，強血尿による貧血で輸血を必要とした。入院同日より高圧酸素療法を開始した。7回終了時には肉眼的血尿はほぼ消失し，計34回の治療を行った。術後10か月目の現在，肉眼的血尿の再発を認めていない。

　A 58-year-old woman with systemic lupus erythematosus（SLE）,which was treated with cyclophosphamide for 7 years（125g in total amount）,was admitted with a chief complaint of massive macrohematuria. Conservative therapy including intravesical was unsuccessful. Therefore,hyperbaric oxygen therapy（HBO）was begun and macrohematuria disappeared after 7 sessions and cystoscopic findings were remarkably improved after 34 sessions of HBO. There were no adverse effects related to HBO. HBO appears to be an effective and safe treatment for hemorrhagic cystitis due to cyclophosphamide.（Rinsho Hinyokika 61：449-451,2007）
