経腰的腎盂撮影法について

TRANSLUMBAR PYELOGRAPHY

緒言

　現在腎疾患の診断には腎盂撮影が最も有力な手段とされている。特に最近の造影剤の進歩および手技の検討，すなわち排泄性腎盂撮影におけるdouble injection method, drip infusion methodおよびdelayed IVPなど長足の進歩が見られ，従来造影不能だつた症例の診断も可能になつた。しかしこれらの排泄性腎盂撮影においても造影が不可能であり，しかも尿管カテーテル挿入不能のため逆行性腎盂撮影も成功しない症例もしばしば経験されるところである。このような場合，経腰的（経皮的）に直接腎盂を穿刺して造影剤を注入し，腎盂および尿管を造影するいわゆる経腰的腎盂撮影法は極めて診断価値が高いと言える。本法は1950年代より報告の見られる比較的新しい方法であるが，直接腎盂を穿刺することの困難さと合併症に対する憂慮のため，主として巨大水腎症に対しては比較的用いられるが，積極的に利用されるには到つていない。

　最近著老も42症例53腎に経腰的腎盂撮影：translumbar pyelography　（以下TLPと略す）を施行し適確な診断を下し得たので，ここに総括して報告する。

Translumber pyelography was performed on 53 kidneys in 42 cases within the 33 months between January, 1967 and September, 1969. 16 kidneys, among the 53 showed definite renal shadow, 36 were indefinite and 1 was unsuccessful, leaving a successful rate of 98. 1%. Among them, 36 kideys were non-functioning, including 14 due to infiltration of carcinoma of uterus to the ureter, 8 renal tuberculosis, of them 2 showed positive acid fast bacilli for the first time by renal puncture, 4 renal calculi, 4 pyonep-hrosis and 2 congenital hydronephrosis, 15 were poor functioning kidneys, including 3 due to tuberculous ureteral stricture,
