腎盂に転移を来たしたVater乳頭部癌の1例

A CASE OF VATER'S PAPILLAR CARCINOMA

緒言

　Vater乳頭部癌は，膵頭部癌に比して，早くから黄疸症状が強く，周辺リンパ節への転移もやや遅いので，3年後生存率も50％近い値を示している。しかし，60歳以上に多いこと，疼痛があまりないこと，治療としては比較的侵襲の大きいWhi-ppleの手術が必要なこと等，全治を困難にさせる素因も多い。転移は大動脈周囲および腸間膜リソパ節，肝，肺に多いといわれる。

　われわれは昭和41年10月にVater乳頭部癌の診断でWhippieの手術を行なつた症例が昭和43年2月（16ヵ月後）に右水腎症を来たし，手術により前記癌の腎盂転移と判つた症例を経験したので報告する。

A 53 years old female, who underwent Whipple's operation for Vater's papillar carcinoma, develo-ped right hydronephrosis 16 months later. Right nephrectomy was performed for right ureteral stricture and pyelonephritis and histopathological findings revealed metastasis of the above carcinoma to the renal pelvis. Such case is rare among the literature and the symptoms and metastatic route were discussed.
