鼠径ヘルニア修復時のメッシュプラグ迷入により生じた膀胱結石

Unusual case of bladder calculus

　鼠径ヘルニア根治術後に生じた稀な合併症であるメッシュプラグ迷入による膀胱結石の症例について報告する。1991年，2003年に両側鼠径ヘルニア根治術（メッシュプラグ法）を他院で施行された既往のある67歳男性。2003年ごろより持続する間欠的な下腹部痛を主訴に当院外科を受診したところ，腹部CTで膀胱結石を指摘され，当科を受診した。経尿道的膀胱結石砕石術を施行し，結石内より鼠径ヘルニア修復時のメッシュプラグと思われる人工物を認めた。後日，人工物を含む膀胱部分切除術を施行した。

　We report unusual case of bladder calculus due to surgical mesh migration used for an inguinal herniorrhaphy. The patient was a sixty-seven years old man,who had several episodes of lower abdominal pain. After the herniorrhaphy. Abdominal computed tomography and cystscopy revealed bladder calculus. We performed transurethral lithotripsy. During lithotripsy,we found surgical mesh in bladder calculus. We stopped lithotripsy,and later we performed partial cystectomy with removed of surgical mesh.
