右腎盂腫瘍，左腎細胞癌の同時発生重複癌

A case of renal cell carcinoma with synchronous contralateral renal pelvic transitional cell carcinoma

　69歳，男性。糖尿病性腎症および右腎結石で経過観察中に，右腎盂腫瘍および左腎腫瘍を指摘された。右尿管鏡下腎盂生検および左腎部分切除術を施行したところ，右腎盂移行上皮癌grade 2および左腎淡明細胞癌（pT1a，grade 2）であった。左側残腎機能が保たれていることを確認した1か月後，後腹膜鏡補助下右腎尿管全摘除術を施行した。病理診断は右腎盂移行上皮癌（pTa，grade 1）であった。同時発生性一側腎盂腫瘍および対側腎細胞癌による重複癌は稀であり，残腎機能に配慮して慎重に治療方針を決定しなければならないと考えられた。

　A 69-year-old man was admitted to our hospital with bilateral incidental renal tumors. Abdominal CT scan revealed left renal tumor and right renal pelvic tumor. Ureteroscopy demonstrated a papillary tumor at the right renal pelvis,and the pathological diagnosis of the biopsy specimen was transitional cell carcinoma. Left partial nephrectomy was performed followed by right total nephroureterectomy. The pathological diagnosis was renal clear cell carcinoma（pT1a,grade 1）of the left kidney and transitional cell carcinoma（pTa,grade 1）of the right kidney. Simultaneously occurred renal pelvic tumor and contralateral renal cell carcinoma is rare and much attention should be paid in choosing treatment strategy.
