尿道進展を認めた高齢者乾燥性閉塞性亀頭炎

Balanitis xerotica obliterans in elderly patients with extension to the urethra

　症例は79歳，男性。他の複数の病院で前立腺肥大と診断され，α1-遮断薬の投薬を受けていた。今回，大量の尿滴下を主訴に当院を受診した。閉塞性乾燥性亀頭炎（balanitis xerotica obliterans：BXO）の診断にて，環状切除術を施行した。BXOは尿道へ進展しており，外尿道口狭窄および尿道狭窄を合併していた。自験例より，老人の泌尿器科疾患は多様であり，注意深い問診および基本診察の重要性を再認識した。

　A 79-year-old male had been diagnosed to have prostatic hypertrophy at other hospitals and had been treated with α1-blocker agents. He presented at this hospital with the chief complaint of uncontrollable terminal leakage of a large amount of urine. Based on a diagnosis of balanitis xerotica obliterans（BXO）,a circumcision of foreskin was performed. In addition,he had meatal stenosis and urethral stricture with an extension of BXO to the urethra. This suggests the importance of careful history taking and physical examinations for a variety of disorders in the field of geriatric urology.
