　外傷を契機に診断された精巣腫瘍の1例を経験したので報告する。症例は56歳，男性。左陰囊部痛を主訴に受診した。触診および超音波検査により精巣上体炎や精巣内血腫を考え，抗生剤を内服したが疼痛は軽快しなかった。診断のため高位精巣摘除術を施行したが，病理診断は精巣腫瘍であった。精巣腫瘍は無痛性の場合が多いが，急性陰囊痛を訴える場合もあり，特に精巣炎，精巣上体炎を合併する場合は，その診断は困難となる。本報告では，急性陰囊痛を主訴とする精巣腫瘍の診断に関して考察を加えた。

　A 56-year-old male presented complaining left scrotal swelling associated with acute and intractable pain after a minor accidental scrotal injury. Ultrasonographic examination, revealed a heterogeneous lesion in the left testis, which we recognized to be intratesticular bleeding or epididymo-orchitis. One week antibiotics treatment did not improve the patient's complaint. Therefore we performed left orchitectomy to obtain pain control and exact diagnosis. The pathological examination revealed mixed tumor combined with yolk sac tumor, mature cystic teratoma and seminoma. Discussion were made on testicular tumors as one of the cause of the acute scrotum.（Rinsho Hinyokika 62：141-144, 2008）
