後腹膜に発生した海綿状リンパ管腫

A case of retroperitoneal cavernous lymphangioma

　症例は66歳，男性。他院の腹部CTで後腹膜腫瘍および肝腫瘤を指摘され，後腹膜腫瘍の肝転移の可能性を否定できず，当科へ紹介された。後腹膜腫瘍は左腎門部腹側に存在する径4cmの腫瘍であり，開腹腫瘍摘出術を施行した。腫瘍は囊胞状で周囲との癒着はなく，容易に摘出された。内容液は乳糜であり，病理診断は海綿状リンパ管腫で，悪性所見は認めなかった。なお，肝腫瘤は血管腫と考えられた。

　A 66-year-old man was referred to us because of retroperitoneal tumor and hepatic mass incidentally detected by abdominal CT. We could not deny the possibility of metastatic retroperitoneal tumor to the liver,therefore open surgical removal of the retroperitoneal tumor was performed. At the operation,the tumor was located just ventral to the left renal hilus and contained chylous fluid and showed no adhesion. The pathological diagnosis was benign cavernous lymphangioma. The hepatic mass was clinically diagnosed as hemangioma.
