尿路に疾患を有する患者における血清シスタチンCの臨床的意義

Clinical significance of serum cystatin C level in patients having urological disease

　尿路疾患を有する110例において，血清シスタチンC（CysC）を測定し，血清クレアチニン（sCr）および24時間クレアチニンクリアランス（24 CCr）と比較した。CysCはsCrに比べ24 CCrとより強い相関を示していた。24 CCrが50～60ml/minの症例では特にその傾向が強く，CysCはより鋭敏に24 CCrを反映していた。正確な腎機能の把握は単腎症例を含む慢性腎臓病の診療や腎毒性を有する薬剤の使用において重要であり，簡便に測定が可能で鋭敏に糸球体濾過率を反映するCysCの有用性は高いものと考えられた。

　We measured the serum cystatin C level（CysC）in patients with urinary tract diseases. CysC reflected renal function more accurately than the serum creatinine level（sCr）and CysC had a more significant correlation with 24-hour creatinine clearance（24CCr）than sCr. Accurate evaluation of renal function is important in the management of patients with chronic kidney disease and for administration of nephrotoxic drugs. Measurement of CysC,which can be done briefly and reflects actual renal function,may be useful in urological clinic.
