頭蓋内血管周皮細胞腫の後腹膜・腎・肺・骨転移

Intracranial hemangiopericytoma with metastasis to the retroperitoneal space,kidney,lung,and bone

　症例は57歳，女性。主訴は腹痛。既往歴は1989年1月脳腫瘍手術。以後再発を繰り返し2度の摘出術とγ-ナイフを施行されていた。2002年1月腹痛を認め近医を受診し，CTにて左腎外側に20cm大の腫瘤を認め当科に紹介された。画像診断にて多発肺転移，対側腎転移，およびS1骨転移を認めた。左腎癌の診断にて経腹的左腎摘除術を施行し，病理検査にて頭蓋内血管周皮細胞腫後腹膜転移と診断された。術後1か月後小脳出血をきたし2002年3月死亡した。

　A 57-year-old female with a 13 years'history of tumor extirpation andγ-knife therapy for intracranial hemangiopericytoma,presented with abdominal pain. MRI revealed left retroperitoneal mass,right renal mass,and S1 bone tumor which were confirmed as metastasis from intracranial hemangiopericytoma after left nephrectomy. Intracranial hemangiopericytoma causes not only local recurrence but systemic metastasis after a long period. Patients with intracranial hemangiopericytoma must be carefully followed up for local recurrence and systemic metastasis.
