上部尿路病変に対する細径軟性尿管鏡の有用性

Endoscopic Management of Upper Urinary Tract Lesions Using Small Diameter Flexible Ureteroscope

　細径軟性尿管鏡は上部尿路病変の診断・治療に重要な役割を担いつつある。今回，われわれの使用した細径軟性尿管鏡は先端部外径6.9Fr．，攣曲角はup,downとも180゜であった。1999年9月より2000年6月までに11例（特発性腎出血3例，上部尿路腫瘍疑い5例，腎結石3例）に対し計13回，細径軟性尿管鏡の使用を試みた。当初の目的を達成し得た成功例は11例中9例で，失敗例は使用直前に軟性尿管鏡の故障が判明した1例と高度尿管屈曲のため挿入が不可能であった1例であった。軟性尿管鏡は操作性に優れている反面，耐久性が低いことを念頭において検査・治療にあたるべきであろう。

Between September 1999 and June 2000 we treated 11 patients with upper urinary tract lesions with 13 retrograde endoscopic procedures using a 6.9Ffiexible ureteroscope.The patients were three idiopathic renal bleeding, fivesuspected upper urinary tract neoplasm and three renalstones before ureteroscopic management,and the manage-ments were successful in 11 of the 13 procedures. In conclu-sion,a flexible ureteroscope with a small diameter wasuseful in the diagnostic evaluation and treatment forupper urinary tract lesion.
