膀胱虫垂瘻の1例

Vesicoappendiceal Fistula:A Case Report

　症例は62歳，女性。繰り返す膀胱炎の精査のため当科に紹介された。膀胱鏡で右後壁に浮腫状で中心部に陥凹を伴う隆起性病変を認めたが，膀胱生検では悪性所見は認めなかった。隆起性病変に対し経尿道的切除術を施行した際に糞臭のする物質を認めた。膀胱造影では瘻孔は描出されなかった。その後下痢と糞尿を認めるようになり膀胱腸瘻の診断にて開腹手術を施行した。虫垂先端が膀胱頂部右側と癒着していたことから膀胱虫垂瘻と診断した。

A 62-year-old woman presented with a 10-year history of recurrent urinary tract infections, disuria, pollakisuria,and gross hematuria. Cystoscopy demonstrated turous lesion in the right posterior bladder wall, which is edematous and indented in the center.Computerized tomography and magnetic resonance imaging suspected primary bladder tumor. There were no abnormal findings by the barium enema radiography and colonoscopy. Transurethral biopsy indicated inflammatory changes without malignancy. After several antibiotics, we found the lesion became smaller on cystoscopy and cystography.
