内分泌化学療法中に両側副腎転移をきたした前立腺癌

Bilateral adrenal metastases from prostatic carcinoma during chemoendocrine therapy

70歳，男性。10年前に多発性骨転移を伴う前立腺癌の診断で内分泌化学療法を開始した。血清前立腺特異抗原（PSA）は359から1.5ng/mlまで低下した。治療開始から4年後にPSAが再燃した。その5年後に腹部CTにて両側副腎腫大が出現した。エコーガイド下に経皮的左副腎針生検を施行し，前立腺癌副腎転移と診断した。治療開始から12年後の現在，副腎以外の部位に癌の再発はなく生存中である。

A 70-year-old man,had been treated by diethylstillbestroldiphosphate（DESD）,etoposide,pirarubicin and cisplatin under diagnosis of prostatic cancer with multiple bone metastases starting in April 1990.Serum prostate-specific antigen（PSA）was reduced from 359 to 1.5ng/ml.Subsequently PSA relapsed 4 years after the initial treatment with the treatment of DESD and Tegafur-Uracil（UFT）.In October,1999,abdominal CT revealed bilateral adrenal mass,which gradually increased in size.Percutaneous needle biopsy of left adrenal gland showed metastatic adenocarcinoma of prostatic origin.Thereafter,a combination of bicalutamide,DESD and UFT was administered.Bilateral adrenal mass decreased in size.He remained well 12 years after the initial treatment without disease progression to any other site except bilateral adrenal glands.
